Assisted living communities help older adults with day-to-day tasks like bathing, dressing, meals, and medication management while keeping them as independent as possible. We track 1,393 assisted living providers across the U.S., with a median cost of about $4,000/month. Medicare does not pay for long-term assisted living, but Medicaid, VA benefits, and long-term care insurance often help cover costs.

Key takeaways

  • Assisted living is for seniors who need daily help but not 24-hour medical care.
  • Median cost in the U.S. is about $5,200 per month, with a range of roughly $4,000 to $8,000+ depending on room type and level of care.
  • Rooms are private or semi-private apartments in a community setting with shared dining, activities, and staff on-site 24 hours a day.
  • Medicare does not pay for room and board. Medicaid coverage varies by state. VA Aid & Attendance helps qualifying veterans and surviving spouses.
  • A good fit is a senior who wants their own apartment, some help with personal care, meals in a dining room, and a social community.

What is Assisted Living?

Assisted living is a residential option for older adults who can no longer manage everything on their own but do not need the round-the-clock medical care of a nursing home. Residents live in private or shared apartments and get help with what are called activities of daily living: bathing, dressing, using the bathroom, moving around, and taking medications. Communities also handle housekeeping, laundry, meals in a shared dining room, transportation to appointments, and a full calendar of social activities. Staff are on-site 24 hours a day. Some communities have a memory care wing for residents with dementia, and many can bring in visiting nurses or therapists for medical needs that come up. Assisted living is regulated at the state level, so the exact services and licensing rules vary depending on where you live.

How much does assisted living cost?

Median: $4,000/month · based on 1,393 assisted living providers we track

The national median for assisted living is about $5,200 per month, but the actual price for any given community depends on room size, the state you live in, and the level of personal care your loved one needs. A small studio in a lower-cost state can be under $4,000. A larger apartment in a high-demand area with heavier care can run $8,000 or more.

Most communities use a base rate for room and board and add care fees on top, either as a flat monthly tier or a per-service charge. Common add-ons: medication management, incontinence care, help transferring in and out of bed, and dementia support. Ask any community for a written breakdown so you know exactly what a monthly bill will look like as care needs increase.

Costs also rise over time. Plan for annual increases of 5 to 10 percent, and for the possibility of moving your loved one to a higher level of care after a fall, hospitalization, or dementia progression.

Who is assisted living right for?

Assisted Living is usually the right fit when your loved one is:

  • Struggling with bathing, dressing, or medication on their own
  • Isolated at home and would benefit from daily social contact
  • No longer safely cooking or keeping up with housekeeping
  • Falling or forgetting medications
  • Not ready for a nursing home and does not need round-the-clock skilled care
  • Family caregivers are burnt out or live far away

Services typically included

  • Help with bathing, dressing, and grooming
  • Medication reminders and administration
  • Three meals a day plus snacks in a shared dining room
  • Housekeeping and laundry
  • Transportation to medical appointments and errands
  • Social activities, outings, and wellness programs
  • On-site staff 24 hours a day
  • Emergency call system in each apartment
  • Some communities: visiting nurse, physical therapy, memory care wing

How to pay for assisted living

Medicare

Medicare does not pay for room and board in assisted living. It will cover short-term skilled nursing or therapy after a qualifying hospital stay, but not the ongoing cost of living in a community. Do not count on Medicare for long-term care.

Medicaid

Most states have a Medicaid waiver program that pays for personal care services in assisted living, though not usually for room and board. Eligibility and covered services vary a lot by state. Ask communities in your state which Medicaid waivers they accept.

VA Aid & Attendance

Veterans and surviving spouses who need help with daily activities may qualify for a monthly Aid & Attendance benefit of about $1,500 to $2,500. This adds to a regular VA pension. Not automatic. You have to apply. A VSO (Veterans Service Officer) can help for free.

Long-term care insurance

Policies vary widely. Some pay a flat daily benefit, others reimburse a percentage of actual costs. If your loved one has a policy, read the elimination period and benefit trigger clauses. Contact the insurer as soon as care needs start to develop.

Private pay

Most families pay for at least part of assisted living out of pocket. Common sources: Social Security, pensions, retirement savings, sale of the family home, and a bridge loan against the home while it sells.

How to choose an assisted living provider

  • Visit at meal time and see what dining actually looks like. Talk to a resident.
  • Ask what happens when care needs increase. Can they stay, or will they have to move?
  • Ask for a copy of the state inspection report and read the complaints, not just the summary.
  • Ask about staff-to-resident ratios during the day, in the evening, and overnight.
  • Ask what is included in the base rate and what triggers a higher care level fee.
  • Trust your gut. If the smell is off, the staff seem rushed, or residents look bored, keep looking.

Assisted Living vs other care types

Assisted Living vs Memory Care

Explore Memory Care

Memory care is a specialized version of assisted living for people with dementia or Alzheimer's. Secured entrances, staff trained in dementia behaviors, and structured programming to reduce agitation. If your loved one wanders, exit-seeks, or has behavioral symptoms, memory care is usually the right fit.

Assisted Living vs Nursing Homes

Explore Nursing Homes

Nursing homes are for people who need 24-hour skilled medical care. IV meds, complex wound care, ventilator support. Most assisted living residents do not need this level of care, and nursing homes cost more (typically $8,000 to $12,000 per month).

Assisted Living vs Home Care

Explore Home Care

Home care brings a caregiver into your loved one's house for a set number of hours per week. Less expensive if you only need a few hours of help. More expensive than assisted living if you need round-the-clock coverage.

Assisted Living vs Independent Living

Explore Independent Living

Independent living is for active seniors who do not need any personal care. Think of it as an apartment community with meals, activities, and maintenance included. If your loved one needs help bathing or with medications, they need assisted living, not independent living.

Frequently asked questions

How much does assisted living cost?

The national median is about $5,200 per month, but the range is roughly $4,000 to $8,000 depending on room size, state, and the level of personal care needed. Most communities charge a base rate for room and board with add-on fees for care.

Does Medicare pay for assisted living?

No, Medicare does not pay for room and board in assisted living. It will cover short-term skilled nursing or therapy after a qualifying hospital stay, but not the ongoing cost of living in a community.

Does Medicaid pay for assisted living?

Most states have a Medicaid waiver program that pays for personal care in assisted living, though usually not for room and board. Coverage varies significantly by state. Check with communities in your state to see which waivers they accept.

What is the difference between assisted living and a nursing home?

Assisted living helps with daily activities like bathing, meals, and medications, in an apartment setting. Nursing homes provide 24-hour skilled medical care for people with serious health conditions. Nursing homes cost more and feel more clinical.

Can my parent bring their pet?

Many assisted living communities allow small pets, sometimes with a size limit, a pet fee, and proof of vaccinations. Ask each community before you tour so you can rule out any that would not accept your loved one's pet.

What happens when my parent needs more care than assisted living provides?

Ask each community about their care levels. Some can handle increased needs on-site by moving to a higher care tier or a memory care wing. Others will require a move to a nursing home. This is one of the most important questions to ask before signing.

How long does it take to move in?

If a room is available and paperwork is in order, most communities can accept a new resident within 1 to 4 weeks. Waitlists are common at popular communities. Assessment visits, financial applications, and a care plan meeting are typically required before move-in.
